No; History : Stray; Subjective: BARH; Observed Behavior – Calm for exam, allowed all handling; Objective ; P = WNL R = WNL BCS 4/9; EENT: Nuclear sclerosis, ears clean, no nasal discharge noted; Oral Exam: Severe dental tartar, gingival recession, gingivitis; PLN: No enlargements noted; H/L: Grade 5/6 heart murmur, sinus arrhythmias, CRT < 2, Lungs clear, no crackles, eupnic; ABD: Non painful, no masses palpated U/G: Male; MSI: Ambulatory x 4, bilateral grade 4/4 luxating patellas, copious amount of fleas and flea dirt, no masses noted, unkempt hair coat, scabbing and lichenification at caudal dorsum; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric; Heart murmur; Dental disease; Luxating patellas; Fleas; Plan: Continue to monitor while at BACC. Scheduled blood work. Rec full cardiology work up with placement; Prognosis: Poor long term prognosis; SURGERY: Permanent waiver due to Age and heart murmur
