Location: Manhattan,  Intake Date: 6/19/18,  Intake Type: Stray

Medical Behavior: Blue,  Sex: Neutered,  Age: 8 years,  Weight: 15 lbs

Original Location: 10458

DVM Intake Exam; Estimated age: Estimated 5-10 years based on dentition and overall impression; Microchip noted on Intake? Scanned POSITIVE; Microchip Number (If Applicable): 0A104E4D4A; History : Stray, no health hx available; Subjective: Calm, walks well on leash; Observed Behavior – Slightly tense or standoffish, allows all handling. Sits down on tail/rear end palpation. Objective ; BAR-H, MMs pink and moist, BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Severe dental tartar, moderate gingivitis, mild gingival recession; PLN: No significant enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Soft, non painful, not distended, no masses palpated; U/G: Male neutered; MSI: Feet are heavily saliva-stained. Small patches of alopecia and scaly skin, surrounded by fur with saliva staining, are noted on the hind legs and especially L and R of the tailhead. Ambulatory x 4, no lameness noted. Skin free of parasites, no masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Perianal skin is alopecic and hyperpigmented. Left anal gland is small and easily expressed. Right anal gland is moderate in size, slightly thickened, easily expressed – discharge is dark brown and thick. Assessment: 1. Pattern of dermatitis – consistent with allergies, R/O environmental vs. food; 2. Dental disease; Prognosis: Good; Plan: 1. Benadryl 12.5 mg PO BID x14 days – can be continued indefinitely if it’s effective 2. Recommend bathing with gentle shampoo weekly, and washing feet or wiping them with antiseptic wipes daily to remove allergens – try this for at least 3 weeks before making a decision about whether it’s effective or not 3. If these measures are not helpful, consider food trial with hypoallergenic or limited ingredient diet 4. Recommend dental cleaning
