Medical Summary  Estimated age: ~8yrs, neutered Microchip noted on Intake? Yes 012538274. Stray dog brought in by field, had been tied to pole  Q/BARH. Allows all handling, attention seeking, tail wagging  P = WNL R = WNL BCS 4/9 EENT: Eyes clear, Mild to moderate otitis externa with purulent discharge, mild seromucoid nasal discharge Oral Exam: mild to moderate dental tartar/gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered MSI: Ambulatory x 4, live fleas, flea dirt along dorsum, small skin tag at right axillary region, generalized pyoderma most significant at ventrum CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, pyoderma, otitis externa, fleas, mild nasal discharge Plan: Ears cleaned, otibiotic applied. Rec otibiotic q12 x 7 days. Doxycycline 300mg PO q24 x 10 days Prognosis: Good
