Vet Notes: 1:44 PM –Reportedly hit by car. S/O -QAR, shy but docile and allows handling -good appetite -mm pk, sl tacky -dermatitis along head, neck -no nasal discharge or sneezing -eupnic, clear lung sounds -ambulatory x 4 but is reluctant to walk due to behavior; prefers to go back in kennel; possible mild lameness on hind limbs but no obvious abnormalities on ortho exam A: 1. Dermatitis 2. h/o trauma P: -simplicef 200 mg PO SID x 10 days -rimadyl 50 mg PO BID x 4 days
Vet Notes: 3:01 PM- SO Reported HBC. no evidence of trauma seen on intake or rounds DVM exam. P is QAR, stoic. Allows all handling. Ears — mild brown debris, AU, AD>AS. skin – live fleas present neurologic — quiet, r/o depressed vs stoic. decreased proprioception in PL, R>>L. r/o stoic vs true neurologic deficit MS – no pain on ortho exam, paraspinal palpation, tail extention / flexion. Decreased cervical range of motion to left (~25% reduction); non painful. Ambulatory x4 with mild ataxia in PL A: HBC, reported by finder otitis externa. geriatric dermatitis dental disease decreased PL proprioception r/o stoic nature vs true neuro deficit (e.g. degenerative myelopathy) P: monitor overnight for respiratory distress. consider radiographs as determined by rounds DVM ear cleaning

Microchip Number (If Applicable): History : Stray – suspect HBC dog Subjective: QAR – did not want to walk and had to be carried. Suspect lethargy vs timid Observed Behavior – timid. Objective: T = 101 P = 80 R = wnl BCS = 3/9. 3-5 % dehydrated EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: upper incisors and 204 missing, all canines worn down/fractured PLN: No enlargements notedH/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Two descended testicles MSI: No fractures or injuries noted on palpation of all limbs, vertebrates and neck. Suspect he did not want to walk due to being anxious vs lethargy small Focal areas of alopecia over body suscpect secondary to scarring/old abrasions Ambulatory x 4, skin free of parasites, no masses noted, hair coat dull CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: externally normal Assessment – no evidence of trauma
1) Emaciated BCS 3/9 2) 3-5% dehydrated 3) Lying down – Paresis vs anxious 4) Dental disease – wore down/fractured canines (no pulp cavity seen). Prognosis: Good Plan: Give all intake txs. Gave 500 ml SQ fluids Monitor in medical overnight and if no issues overnight then send to adoptions the following day. SURGERY: Permanent waiver due to age
