Medical Summary DVM Intake Exam 6/16/17 Estimated age: ~12yrs, male Microchip noted on Intake? No History : Stray Subjective: Observed Behavior – QAR. Allows all handling. Offered food, ate immediately but with difficulty Evidence of Cruelty seen – Yes Evidence of Trauma seen – Yes Objective T = P = R = BCS EENT: Nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Severe dental tartar/gingivitis causing significant gingival recession, rotting and necrosis of upper and lower jaw PLN: Moderately enlarged submandibular LN H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Non-weight bearing RH limb, severe matting at exremeties, open wound at lateral right rock, severe matting and urine staining around penis CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Severe neglect causing necrosis of jaw and strangulating lesion at RH limb – bone likley necrosed Plan: Rec placement for hospitalization and wound care, following up with cruelty group now will schedule pain management and abx Prognosis: Fair with appropriate treatment SURGERY: Permanent waiver due to age and medical conditions
