DVM Intake Exam; Estimated age: 7yo; Microchip noted on Intake? neg; History : Stray; Subjective: BARH. Caution, tried to bite without warning, while head was being petted. Previously p appeared calm, friendly, curious, and wagging tail. Growling and lunging while trying to place muzzle. Muzzle was placed and p remained tense and growling. P must have bit lip, slight bleeding from mouth. Limited PE due to behavior. Observed Behavior – Objective ; T =; P = wnl; R = eup; BCS 6/9; EENT: Eyes clear, no nasal or ocular discharge noted; Oral Exam: unable to examine; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: not assessed due to behavior; U/G: MI, 2 testicles; MSI: Ambulatory x 4, no ectoparasites seen, no masses noted, fair hair coat. Lichenification along both elbows, alopecia noted on dorsal left antebrachium; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; intact; alopecia — r/o allergies vs ectoparasites vs other; bleeding from mouth — r/o self induced trauma; Prognosis: good; Plan: plan for neuter; behavior eval; SURGERY: Okay for surgery
