Medical Summary Estimated age: 12-14 years Microchip noted on Intake? placed on intake. stray, easy going and allows handling BCS 6/9 EENT: OU-mature cataracts, iris atrophy nose clear, ears clean Oral Exam: mm pk/pigmented; severe dental disease PLN: No enlargements noted H/L: grade III/VI heart murmur; eupnic, normal lung sounds in all fields ABD: sl pot bellied appearance; soft, nonpainful U/G: male intact; one testicle is much larger than the other but palpates normally MSI: small (<0.5 cm) tan hairless growth over R eye ambulatory x 4 with mild proprioceptive ataxia in hind end no significant back pain crepitus and mild thickening of R stifle hyperpigmentation on abdomen CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Cataracts Dental disease heart murmur-r/o valvular vs other small growth above eye-likely benign mild ataxia-suspect IVDD due to breed Plan Rec’d CBC/chem Rec’d cardiac evaluation-chest xrays minimally but ideally would follow up with echo, blood pressure monitoring and ECG Rec’d dental cleaning with likely extractions Rec’d neuter after cardiac evaluation Small growth above eye is likely benign but can be removed if already under anesthesia for dental Monitor for back pain that may be associated with IVDD Prognosis: fair to good SURGERY:permanent waiver d/t heart murmur
