10/17/16 : Geriatric exam
O: BARH. mm=pink, moist, CRT<2 s. Sweet, allows all handling. ORAL: Severe calculus, gingival recession, missing incisors. Mand canine teeth protruding. EENT: Nuclear sclerosis OU. H/L: Gr II/VI left sided cardiac murmur. Lungs clear. ABD/UG: palpation WNL. MS: Amb x 4. No palable abnormalities. INTEG: 2-3 mm raised pink growth dorsum of head and doral R. thorax. 1 cm raised pink growth L. lat flank. A: Severe dental disease. Cardiac murmur – r/o mitral valve disease. Skin masses – r/o sebaceous adenomas. P: Rec. dental cleaning/treatment – expect he will need many extractions. Advise workup (blood tests, cardiac) prior to anesthesia. Fair to good prognosis with appropriate management.
PROFILE :
Peluche is a male white Shih Tzu that is 12 years old. The owner received Peluche as a gift. The owner is surrendering Peluche due to having no space in the home. Peluche has lost some of his food.
Peluche has mostly lived adults and his friendly and affectionate. Peluche has been introduced to children ages 12 and up. He is relaxed and keeps to himself. When stranger come over Peluche sniffs people and solicits attention. He has never lived with dog or cats. He is relaxed around dogs in the neighborhood.
Peluche allows being bathed and having his nails trimmed. But the owners have not done it for a while. Peluche has never had a bite history. He is walked 3 times a day but has accidence around the house. He allow for people to touch his food bowl while eating or removing toys from his mouth.
Peluche is described as mellow and friendly. He sleeps throughout most of the day. He does not care much for toys. When the family is in the living room he follows and lays down next to the sofa. He enjoys a diet of Cease wet food 1 a day. Sleeps on his dog bed in the owner’s bed room.
Peluche during intake was relaxed and friendly. He allowed being collard, scanned and photographed.
PLAYGROUP:
Peluche’s previous owner reported that he is tolerant of the neighborhood dogs.
10/17: When off leash at the Care Center, Peluche greets other small male and female dogs but mostly just keeps to himself.
