MEDICAL:

VC – GERIATRIC EXAM, CHECK RECTUM AND EYES  S/O: Dog is QARH/depressed  BCS 3-4/9  EENT – OD sunken with mucoid discharge, OS protruding with no discharge but cornea cloudy with neovascularization; STT OD-7mm/min OS-19mm/min  Grade 3-4/4 dental dz; mm – pink, moist; crt < 2s Grade IV/VI heart murmur  Approx 2-3cm x 4-5cm ulcerated mass to right of rectum; does not communicate with colon but feels like it may be associated with anal sac

A: Ulcerated perianal mass DDx: Adenocarcinoma, Anal Sac vs Perianal, Open; KCS OD

P: Informing NH to give deadline of 7pm tonight, if not placed recommend EHR due to poor px and underlying conditions

PROGNOSIS:  Poor-Guarded

PROFILE:

Basic Informtion:  Peluche is a fourteen year old shih tzu who was surrendered by his owner due to his owner moving into a home that does not accept pets. He is described as shy, great with kids and mellow. He was with his owner for fourteen years and was brought from a breeder as a puppy.

Socialization:  Peluche has lived with two adults and two small children. Around children he is described as relaxed and tolerant; he plays gently. He lived with two other dogs in the home and is described as relaxed and respectful around dogs. He will engage in gentle play. He has not been around cats. He is mostly houstrrained but due to his age has had accidents in the home more frequently. He has been kept only indoors and is activity level is described as low.

Behavior: Peluche is not bothered when his food is touched while eating or when a treat is taken away. He is fearful when bathed or when his nails are trimmed and will struggle. Owner stated he enjoys being brushed and will growl when held. Owner stated he does not like to be picked up. He is friendly when strangers approach his home and will greet the visitor with a loose and calm body.

For a new family to know: Peluche knows the commands sit, down, stop and stay. He is sued to slow leashed walks once a day, owner stated he has issues walking for longer periods of time and will get tired easily. He likies squeaky toys and playing fetch. He will go to the bathroom on newspaper, pee pads and on cement. When home he tends to be in a favorite spot, and is well behaved when left alone. When off leash he will stay close to his owners side. He is not bothered by loud noises. He eats dry and wet food Purina dog chow and is free fed. He sleeps in his favorite spot.

Upon intake Peluche was whining and had a tense body, he allowed all handling. Counselor was able to collar, scan (negative) and photograph.
