Medical Summary DVM Intake Exam Neutered male Estimated age: 7 years Microchip noted on Intake? negative History : Patient is stable. Acting lethargic. Not walking Subjective: Neutered male , trouble walking dog Observed Behavior – Allows handling but do not toleraed anybody touching his fore limbs paws. He gets very anxious and might bite if his fore limb paws are touched. Objective T = 101.5F P = 130 bpm R = 24 bpm BCS 5/9 EENT: Eyes- while “film” in both corneas, ears clean, no nasal discharge noted Oral Exam: moderate dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered male MSI: Non ambulatory in the hing limb, could not detect any crepitus or abnormalities in the jointrs, skin free of parasites, no masses noted, Some areas of matted hair. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NAF Fecal test- nps bloodwork- pending Assessment Trouble walking- ostheoarthritis vs neurologic vs other Plan 200 ml LRS once No treatment at this moment Radiography is indicated Prognosis: fair
