Subjective:BARObserved Behavior – barking excitedly in kennel for attention; once out ofkennel, she is friendly and docile and allows all handling. Objective: BCS 5/9 EENT: OU crusting discharge along medial canthus, AU-mild erythema, severe dark crusting discharge; no nasal or ocular discharge noted. Oral Exam: mm pk, moist; CRT <2 sec; severe dental tartar PLN: No enlargements noted H/L: eupnic, clear lung sounds, no murmurs/arrhythmias ABD: soft, nonpainful, no palpable abnormalities. U/G: female intact, no spay scar or tattoo noted; prominent nipples. MSI: -diffusely matted hair coat, no obvious irritation secondary to matting -bilateral MPL, grade III-IV/IV LH, grade II-III/IV RH. CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal:grossly normal. Assessment: 1. Geriatric 2. Dental disease 3. Patellar luxation, bilateral 4. Otitis externa. Prognosis: fair to good Plan: -flushed ears during intake -gentizol AU SID x 7 days -recommend shavedown -recommend dental cleaning after placement -recommend weight management through diet to protect joints -monitor for significant lameness, patellar luxations do not appear painful or causing significant lameness at this time but if lameness does occur, then consider surgical stabilization of L stifle SURGERY:permanent waiver due to geriatric
