Head shy; Objective ; P = WNL R = WNL BCS 5/9; EENT: Nuclear sclerosis OU, ears clean, no nasal discharge noted; Oral Exam: Moderate to severe dental tartar/gingivitis; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Male, left abdominal cryptorchid, right testicle soft; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric, dental disease, abdominal cryptorchid; Plan: Continue to monitor while at BACC. Rec dental cleaning +/- extractions with placement; Prognosis: Good; SURGERY: Permanent waiver due to age
