Medical Summary 10/9/16 Check ulcerated mammary tumor O: BARH. mm=pink, moist, CRT<2 s. Calm, easy to handle and examine. ORAL: Mod. calculus, gingivitis. EENT: No oculonasal discharge. Lichenification AU, mild tan discharge. ABD/UG: Palpation WNL. INTEG: Multiple mammary tumors: Left side – 3 cm diam. mass 1st mammary gland; 1 cm mass between 2nd and 3rd; Right side – 1 cm mass between 2nd and 3rd; 6 cm x 4 cm ulcerated mass 4th mammary gland. Ulcerated area is moist, not currently bleeding. Not painful on palpation. A: Chronic otitis. Multiple mammary masses, with largest one ulcerated. R/o mammary adenocarcinoma. At risk for infection. P: Rx Amoxi-clav. 45.7 mg/ml. 1.0 ml PO BID x 10 days. House in medical area to monitor for any complications from ulcerated mass. Will need full workup when placed – including blood tests, chest rads prior to surgical therapy. Fair to poor prognosis.
