Medical Summary DVM Intake Exam Estimated age: 12 yrs old MC placed at time of LVT intake History : stray brought in Subjective: QAR to slightly depressed mentation, weak, allows all handling Observed Behavior – lying down, weak Evidence of Cruelty seen – malnourished, reported to be stray Objective heating pad on T = 98.8F P = 120 R = 30 BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: moderat tartar, pink gums, recessed gingiva at maxillary canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: grossly normal, no MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: dark stool with blood on glove Assessment dehydrated, emaciated, melena (r/o GI bleed, GI neoplasia, other), weak, inappetent, moderate dental and periodontal dz Plan IV catheter and LRS 12ml/hr CBC/chem in house- neutropenia with bands suspected, GLU 130, elevated ALK PHOS 626, elevated PHOS 11.7, BUN >130, BUN unable to read: r/o CRF versus pre renal from dehydration, underlying infection, neoplasia, underlying metabolic condition, other check app mirtazipine 15mg: 1/4T PO SID x 3 sucralfate in slurry BID x3 pending test results consider 3v met check, AUS Prognosis: open **addendum: approx 12 noon reported to have finished food and drinking a lot of water. SURGERY: waived Permanent waiver due to age, and emaciation

Who does not love Ms Panini? She is a hit at the first encounter…Who would think? An elderly gal, skin and bone, dehydrated… She caught the heart of everybody around and gets tender loving care from the medical folks. We, volunteers are allowed to visit and we do not miss an opportunity to take Ms Panini for a short and slow walk in the yard. She is timid and gentle and we are handling her like she is made of the most fragile material…She is such a brave and smart little soldier, trying her best to stay afloat and dealing with whatever we are proposing to her like the great trooper she is. Ms Panini deserves the best, a welcoming home and a loving master. She might be twelve years of age but might enjoy a few more in caring and savvy hands…Ms Panini is at the Manhattan Care Center, wishing upon a star….
