DVM Intake Exam; Estimated age:8 reported; Microchip noted on Intake? n; History : o surrender; Subjective: Observed Behavior -calm, allows exam; Objective T = P =100 R =wnl BCS 5/9; EENT: sclerosis ou, ad>as hyperkeratosis, no nasal or ocular discharge noted; Oral Exam:minor staining; PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated U/G: intact male; MSI: Ambulatory x 4, significant skin disease: rear feet pododermatitis, alopecia and erythrm interdigitally. periocular dermatitis; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: skin allergies likely, malasezzia component likely; Prognosis: good, but condtion may be chronic Plan: antibiotic trail and mometamax. diet trial in future; neuter SURGERY: Okay for surgery
