Medical Summary 05/08/16 17:32 Scan : negative Sex: female Approx 7-10yr S/O: BAR, nuclear sclerosis OU. Dental disease, brief exam only, snapped during examinatino of mouth. Clean ears. Two area of scabbing rostral to each eye, on maxilla. Normal cardiothoracic auscultation. Cleaned scabs away w/ dilute chlorhexidine, reveal draining tracts, small amount of purulent exudate. A: Geriatric, severe dental disease, presumptive tooth root abscesses. P: Recommend NH plea for placement for complete oral exam, dental prophylaxis, likely extractions. Recommend CBC/Chem/UA prior to anesthesia Permenant wiaver from ACC due to age and medical condition. PROGNOSIS: GOOD -FAIR
