Medical Summary microchip scan positive 457327607E Estimated age: 14 yrs. Stray dog, transferred from SIACC on 4/8/17. Noted to be blind, walking slowly, had overgrown nails, and has several masses. Sleeping when approached in cage, slow to rouse. Woke up when food placed in front of him. Very quiet, calm, and gentle dog. QARH. mm=pink, moist, CRT<2s. HR=80, RR=12. BCS=4/9. ORAL: Severe calculus, mod. gingivitis, multiple areas of gingival recession. EENT: AU – mild waxy debris in outer portion of ear canal, vertical and horizontal canals clean and WNL. OU – 100% corneal opactification, nictitans appears adhered to ventromedial cornea OU. No discharge, no pain on retropulsion of eyes. No nasal discharge. H/L: Lungs clear, no coughing. No dyspnea. Gr 2/6 left sided systolic murmur. ABD/UG: Soft, non-painful, no masses/organomegaly. Neutered male. MS: Reluctant to rise until enticed with food. Slowly amb x 4, no lameness, no pain on limb or spine palpation. Left hind, digit 5 – mod. swelling around distal toe and pad, with 5 mm wound and purulent discharge. INTEG: Medial left thigh – 6 cm diam soft, mobile SQ mass. Lat left elbow – 1 cm pink, raised skin mass. Lat right shoulder – 5 cm x 3 cm x 3 cm soft skin/SQ mass. NEURO: Quiet but appropriate mentation. Appears to be deaf. No other obvious neuro deficits. A: Geriatric dog – age est 14 years. Severe dental/periodontal disease. Cardiac murmur – r/o mitral valve insufficiency. Infection in L hind digit 5 likely from overgrown nail that traumatized pad. Corneal opacification – r/o prior corneal injury or infection resulting in permanent changes and blindness; no active disease present. Reluctance to walk – r/o due to blindness, osteoarthritis, discomfort from infected toe, spinal problems. Medial thigh mass – suspect lipoma. Elbow mass – r/o mast cell tumor, adenoma, other. R. shoulder mass – r/o mast cell tumor, cyst, basal cell tumor, sarcoma, other. P: Rx Rimadyl 100 mg 1 PO q 24 hours x 5 days. Rx Clavamox 375 mg 1 PO BID x 10 days. Schedule for CBC/CHEM panel. Fair prognosis with appropriate management.
