Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective : P = wnl R = tachypnea, dyspnea, RR 42 bpm BCS = 4/9 MM pale pink, CRT <2 EENT: Eyes cataracts OU, ears clean, no nasal or ocular discharge noted Oral Exam: mod-severe ddz PLN: No enlargements noted H/L: Grade V/VI HM, CRT < 2, Mild crackles ausculated bilaterally , abdominal effort ABD: Severe distention, non painful, no masses palpated MSI: Stands x 4, but reluctant to walk, pitting edema in all four limbs, marked DMW, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Diarrhea in fur around anus Assessment: Suspect CHF , Pitting edema, Suspect Ascites, Heart murmur, Diarrhea, Cataracts Prognosis: Guarded to poor Plan: Furosemide 6mg/kg IV, Furosemide 2 mg/kg PO Q4 hours Recommend CBC/Chem, TXR If dyspnea/tachypnea do not improve SURGERY: Permanent waiver due to age
