10Y Neutered Black/White Small Mixed Breed 20lbs
Owner Surr:7/14/2018

DVM Intake Exam; Estimated age: 10yrs based on O hx; consistent with exam.  Microchip noted on Intake? scanned positive by LVT on intake. MC# 985121004354150; History : Surrendered by O.  Subjective / Observed Behavior – BAR, allows all handling.  Objective ; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: dc 2/5; pd 2/5 ; PLN: No enlargements noted; H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: neutered male.  MSI: Ambulatory x 4, skin free of parasites, healthy hair coat. multiple haired, dermal, freely moveable masses throughout body ranging from 0.5cm to 2cm.  CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal.  Assessment; dermal masses ; keratinic cysts ; dental disease; blind; geriatric; diarrhea -r/o dietary indiscretion, giardia, intestinal parasites, stress ; Prognosis: fair; Plan: 2 cytologies (right lateral thigh [dorsal mass] and dorsal lumbar masses) — keratin, adipocytes, debris.  ok for adoption!  metronidazole 250mg tablet — give 0.5 tablet PO q12h x 5 days; proviable — give 1 capsule q24h x 5 days ; SURGERY: already neutered

Let’s get to know each other a bit more…

Basic Information:  10 year old Spanish speaking Oreo Cakepop was surrendered because he is blind and uses the bathroom in the house. His family got him from a neighbor 10 years ago. His owners reported that they think he has arthritis because he doesn’t like his feet touched.

Previously lived with:  3 adults, 1 child: 16
How is this dog around strangers?  Oreo Cakepop is friendly around strangers.
How is this dog around children?  Oreo Cakepop has spent time with a 16 year old for the last 10 years. He is very friendly and gentle with them.
How is this dog around other dogs?  Oreo is relaxed around other dogs.
How is this dog around cats?  Oreo has never been around cats.
Resource guarding:  Oreo isn’t bothered if you take away his food, treats, or toys. When someone unfamiliar approaches the home he growls a little but stops when you tell him to.
Bite history:  Oreo has never bitten a person or animal.
Housetrained:  Partially
Energy level/descriptors:  Medium

Other Notes:  Oreo goes to the bathroom indoors because he is blind. He is not bothered by storms or fireworks. He isn’t bothered by having a bath or being brushed. He doesn’t like having his paws touched during nail trims possibly because of suspected arthritis.

Has this dog ever had any medical issues?  Yes
Medical Notes:  Oreo is blind, had his right eye removed, possible arthritis, and two bumps on his back.

For a New Family to Know:  Oreo is described as affectionate, playful, confident, and excitable. He loves sitting on the couch with you, watching television, and jumping on the bed with children. He is mostly indoors. He slept in the bed with his former owners’ grandchild. He eats 1 can of wet food once a day. He enjoys doggy biscuits and bread. He is accidents daily because he is blind. He is well-behaved when left alone. He’s never been crate trained. Oreo speaks Spanish and knows sit. He doesn’t go on walks anymore.

Behavior Notes:  Oreo Cakepop scanned positive for a microchip. He bumped into walls and people during the intake. However, he was still wagging his tail. He allowed the admissions counselor to pick him up and take his picture.

Details on my behavior are…

Date of assessment:  16-Jul-2018
Look:  1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity:  1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag:  1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.
Paw squeeze 1:  1. Dog gently pulls back his/her paw.
Paw squeeze 2:  1. Dog gently pulls back his/her paw.
Toy:  1. No interest.

Summary:  Oreo Cakepop approached the assessor with a soft body. He allowed all handling and displayed no concerning behaviors.
Summary (1):  7/16-17: When off leash with dogs, Oreo Cakepop greets politely and then wanders the yard mostly keeping to himself.

BEHAVIOR DETERMINATION:  AVERAGE (suitable for an adopter with an average amount of dog experience)
