05/19/17 Monitor dog with severe pyoderma — check for scratching S/O: QAR. Trembling INTEG: Severe erythema at ventral right side of neck, generalized dermatitis/pyoderma (full description from intake below) A: Not observed scratching by DVM or ACS — senior pet with allergies P: Adding benedryl 25mg PO q12 x 5 days. Has been on Cephalexin and Apoquel, but no recent veterinary records or exam findings presented. Subjective: Observed Behavior – Shy, trembling, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: QARH. mm=pink, moist, CRT<2 s. HR=116, RR=20. BCS=4/9. ORAL: Mild calculus. EENT: No oculonasal discharge. 05/10/17 11:36 Oreo is a nine year old female Jack Russell terrier. Oreo was brought from a friend and was with her owner since she was a few months old. Oreo is being surrendered by her owner due to cost of care due to Oreos rising medical issues as well as owners own health. Oreo suffers from severe skin allergies and chronic ear infections. She was taken to Animal Clinic of Canarsie in Brooklyn for wellness and checkups. Oreo has lived with four adults in her previous home. Oreo is described as protective of her family members and will growl and lunge at strangers. She was usually kept in the crate when strangers visited the home. Owner stated Oreo will usually bark while in the crate. Oreo has not been around children or cats. Owner stated she had limited interactions with other dogs and would usually growl and keep her distance. Oreo has never bitten in her previous home. Oreo barks a lot in the home, owner stated she is very vocal and lets her presence be known. Oreo does not like to go outside for walks and was provided pee pads in the home. Oreo will drag her body and not walk on leash. Oreo is not afraid of loud noises like storms or fireworks. Oreo was fed Salmon flavored dry food due to possible food allergies; owner fed her half a cup twice a day. Oreo loves peanut butter treats. Oreo does not mind if her food is touched while eating or if she is approached. Oreo will bark and whine when left alone in the home according to her owner; she likes to stay with the family at all times and can be found following someone around the home. Oreo knows the commands sit and stay. She loves to play with plastic bottles and running and chasing everyone around the home. Oreo has a medium to high activity for an older dog states her owner. Oreo is fearful of water and being bathed and will flee and run around the home; once she is caught and given a bath she will relax and allow it. Oreo will growl and wag her tail when a toy is taken away and owner stated she tries to play tug with the toy.
