Medical Summary Estimated age: 3 years Microchip noted on Intake? positive, 981020019776549. Came in as a stray. Microchip positive-traced back to the owner (was adopted from ACC). BAR, Very friendly, loose, and wagging his tail for PE. P = wnl R = panting, eupneic BCS 3/9 EENT: Eyes clear, ears mild to moderate brown waxy debris with very short cropped pinna, no nasal discharge noted Oral Exam: mild dental tartar, prognathism PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, pink, moist, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN, no d/c MSI: Ambulatory x 4, skin free of parasites, no masses noted, haircoat is dry and brittle. He has diffuse alopecia and scaling and crusts. Diffuse erythema and very pruritic. He has a small circular ~2cm abrasion on his cheek caudal to his left ear. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Suspect pyoderma r/o allergies Weight loss r/o starvation vs underlying health condition Plan: Cephalexin 30mg/kg PO BID x14 d benadryl 2mg/kg PO BID x14d Recheck skin and ears in 7 days-if not improved recommend skin scrape, cytology CBC/Chem: ALP low <10 (23-212), hypochloremia 108 (109-122), cbc wnl Clean ears Prognosis: good SURGERY: neutered

Medical Summary No known history – found tied up along street around a lot of glass. ACC driver reported bleeding from mouth, therefore brought to medical. Subjective: Timid but gentle. Allowed most of exam. Some blood observed on left front paw. Patient appeared hesitant, but curious. With slow handling and approach, patient will allow most of exam without restraint. Certain times (drawing blood, cleaning ears), patient required extra restraint T = P = 180 bpm R = panting BCS = 4/9 EENT: Mild hyperemic conjunctiva OU, Moderate amount of tan, moist, malodorous debris AU with mildly erythematous canals, no nasal discharge noted Oral Exam: Mild, thick serosanguinous discharge along buccal mucosa, bilaterally. Unable to determine site of active bleeding. Mild dental tartar. Small mucosal abrasion above tooth 204. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, no obvious ectoparasites, no masses noted, Small abrasion just proximal to right cranial stifle, small abrasion on dorsal right front paw, lateral aspect CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed Assessment: Approx. 3 yr. old MI Eng.
