Medical Summary DVM Intake Exam Estimated age: 8yrs Microchip noted on Intake? neg History : stray Subjective: friendly Observed Behavior – allows all handling Objective P = 120 R = 36 BCS 6.5/9 EENT: Eyes clear, ears clean, no nasal discharge noted, Oral Exam: oligodontia, two lower canines intact, caudalmost upper molars- severe dental tartar, tongue sticking out, couple of spots along upper gum line where maxillary bone visible PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: no visible tattoo or spay scar, approx 3cm firms SQ mass associated with R caudalmost mammary gland MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment MGT: r/o benign vs. malignant severe dental dz and oligodontia, areas of maxillary bone visible overweight HM: r/o valvular, myocardial Plan rec dental with extractions, rec lumpectomy and histopathology of MGT- 50/50 chance benign/malignant in dogs rec ideally ECHO prior to any ax, 3v met check prior Prognosis: open pending bx results SURGERY: Permanent waiver due to MGT and HM
