Medical Summary DVM Intake Exam Estimated age: 3-5 years based on dentition and overall appearance Microchip noted on Intake? Scanned positive History : DOH-B – VERY dog-reactive but great around people. Subjective: Panting, relaxed body, allows all handling. No growling or other sign of aggression. Objective BAR-H, MMs pink and moist, BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Mild tartar on PMs and Ms PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male neutered MSI: Decreased extension in both hips but especially right hip, no sign of discomfort on extension. Skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Mild dental disease 2. Decreased ROM in hips but no sign of pain on extension Plan: Recommend daily tooth brushing, monitor gait and consider pain meds in the future if necessary Prognosis: Good 1088
