Medical Behavior: Blue Sex: Female Age: 10 years

DVM Intake Exam : buphthalmic OD, diagnosed with glaucoma (IOP 44 OD, 18 OS) Started on Dorzolamide/Timolol BID and Buprenorphine 0.02 mg/kg IM BID x 3 days. CBC/Chem/T4 CBC- PLT 508, otherwise unremarkable. Chemistry unremarkable Objective: P = wnl R = wnl BCS 7/9. S: BARH. Nervous, but friendly. Likes to be held and able to handle easily. No csvd. Moderate amount of urine in cage, formed stool. Ate very well EENT: OD- Moderate bupthalmos, mature cataract with apparent suture lines noted, moderate to severe neovascularization of the cornea with mild to moderate edema noted. Normal OS. ears clean, no nasal or ocular discharge noted. Oral Exam: severe dental tartar/periodontal. disease noted. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, pulses adequate bilaterally. Lungs clear, eupneic. ABD: Non painful, no masses palpated. U/G: intact female with prominent vulva, no discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. CNS: Mentation appropriate; non-visual OD, normal OS. Rectal: normal externally IOP: 35mmHg OD, 17mmHg O Assessment: 1) Glaucoma OD, secondary to cataract (likely luxated lens or breakdown of lens) 2) Severe dental disease. 3) Geriactric. 4) Obese Prognosis: Fair with proper management Plan: Continue Dorzolamide/Timolol OD BID. Continue Buprenorphine 0.02mg/kg IM BID until 5/30, then reassess. Recheck IOP daily until pressures improve and are consistent. Seek NH. placement – recommend opthalmology consult (and possible cataract surgery vs medical. management) vs enucleation. Can move to adoptions SURGERY: Permanent waiver due to age
