Medical Summary DVM Intake Exam Estimated age: 3yr Microchip noted on Intake? none History : o surrender, stated that couldn’t feed p anymore, has other dogs Subjective: docile, sweet, readily eats well- VERY HUNGRY Observed Behavior – allows all handling Evidence of Cruelty seen – YES Evidence of Trauma seen – YES Objective P =120 R = 30 BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: pale pink gums, mild staining and some green faint marks on canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, grossly normal, no palpable MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, pressure sores over hip bones, L scapula acromion, overgrown nails, R FL- almost entire large footpad appears cut off- flesh has some debris and possibly necrotic tissue on it, moderate swelling of the distal foot up towards the metacarpals. Also scabs and spots of alopecia on cranial aspect of leg (scars, other) face- multiple areas of alopecia with mild scabbing RH- linear thin area of alopecia- appears as if something were tied around leg for a prolonged period of time both hocks also have pressure sores CNS: mentation appropriate – no signs of neurologic abnormalities Assessment HIGH SUSPECT CRUELTY wounds, emaciation suspected that large footpad on R FL was cut or bit off, secondary swelling on leg appears that a ligature/string/rope was around the R HL for a prolonged time Plan Wood’s Lamp negative gently cleaned footpad with diculted chlorhexadine solution, placed modified RJ bandage feed TID clavamox 125mg: 1T PO BID x14 tramadol 50mg: 1/4T PO BID x2 for pain associated with wound check app took photos and sent to Cruelty group, alerted Cruelty group Prognosis: good, if footpad doesn’t heal may require some debridement SURGERY: Temporary waiver due to severe emaciation, wounds
