Medical Summary DVM Intake Exam Estimated age: 8 years based on dentition Microchip noted on Intake? Scanned neg History : Stray, reportedly tied to a fence for a week Subjective: Alert, standing, walking around Observed Behavior – Wagging tail, allows all handling Objective BAR, BCS 1-2/9, MMs pink and moist EENT: Moderate brown crusty debris surrounding OU, mild conjunctival erythema. Clean ears and nose. Oral Exam: Severe tartar and gingivitis, gingival recession PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Tucked up, non painful, no masses palpated U/G: Male neutered, no testicles palpated MSI: Small (1 cm dia) pink alopecic dermal mass on dorsal pelvis, just to the side of the tailhead. No surrounding swelling or erythema. Ambulatory x 4, emaciated, generalized muscle wasting, skin free of parasites, healthy hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Schirmer tear test: OS 15 mm/60 sec, OD 7 mm/60 sec Assessment: 1. KCS OU 2. Emaciated 3. Mass on dorsolumbar region 4. Dental disease Plan 1. Optimmune OU BID **indefinitely** to treat KCS 2. Recommend weight gain – okay to hold off on microchipping for a few days 3. Recommend FNA of mass, consider excisional biopspy 4. Recommend dental cleaning followed by regular tooth brushing Prognosis: Fair 1088
