Medical Summary DVM Intake Exam Estimated age: Reported 9 years – exam is consistent with this Microchip noted on Intake? Scanned negative, MC placed during exam History : No medical hx given Subjective: Barks repeatedly when left alone in kennel. Allows full exam with soft body. Struggles a little on blood draw. Objective BCS 4.5/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Severe tartar PLN: Mandibular LNs are moderately enlarged, but symmetrical. Other LNs are normal. H/L: NSR, NMA, CRT < 2, Coughed several times at the beginning of the exam. ABD: Non painful, no masses palpated U/G: Male intact. Left testicle is inguinal cryptorchid. MSI: Medial patellar luxation – left stifle grade 2/4, right stifle grade 1/4. Skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment 1. Dental disease 2. Cryptorchid 3.
