Medical Summary Estimated age: 1.5 Microchip noted on Intake? no. O surrender. has been sick for at least 6 months. lethargic, emaciated. partial work ups at several RDVMs not conclusive but point towarrds panreatitis/pancreatic insufficiency +/- liver shunt took a course of prednisone in 2/17, which may explain elevated ALP . QAR P = 50 R = wnl BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: palu gums, clean teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: open ddx: 1-exocrine pancreatic insufficiency, possibly with active panreatitis (may be a sequelae to prev pancreatitis) 2- liver shunt 3-chronic blockage gi tract (less likely) 4-other GI illness Plan this dog NEEDS the following tests: 2 sample bile acid profile, maldigestion profile including TLI, abdominal ultrasound/shunt hunt, repeat cbc/chem Prognosis: guarded. if my suspicions are correct, this dog is treatable but may require lifelong meds or surgery. SURGERY: Permanent waiver due to illness
