Medical Summary 05/15/16 14:51 initial exam microchip scan: negative weight: dull mentation, very twitchy/jumpy on exam, severely ataxic – can take a couple of steps and then will fall over approx 3 year old female dog emaciated body condition (BCS 1/9) matted haircoat but no wounds seen mm’s pink and moist, mild dental tartar build up, adult dentition clean ears AU no ocular or nasal discharge, no CIRDC signs heart and lungs WNL CBC – WNL Chemistry – severely elevated ALT (>1000), otherwise WNL ***suspect toxicity due to neurologic signs with severely elevated ALT but otherwise bloodwork WNL*** attempted to place IVC – unable start LRS BID rec NH placement – likely needs IVF/supportive care/follow up blood work prognosis: fair/good if shows improvement, poor if neurologic signs persist/worsen
