DVM Intake Exam; Estimated age: 10 year; Microchip noted on Intake? none; History : stray found on street; Subjective: Observed Behavior – allowed all handling. sought human attention. no reaction to loud noises; Objective; T =; P = 60; R = 24; BCS 4/9; EENT: Eyes clear, immature cataracts in both eyes, menace positive OU, ears clean, no nasal or ocular discharge noted. no reaction to loud clapping behind each ear; Oral Exam: severe bridging calculus and severe gingivitis with severe halitosis. several teeth missing; PLN: No enlargements noted; H/L: left systolic grade II heart murmur, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: spay scar; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: clean on external exam; Assessment:geriatric. severe grade IV dental disease and oligodontia, grade II heart murmur, bilateral immature cataracts, deaf, underweight; Prognosis: fair; Plan: recommend for adopter to follow up with veterinarian and get teeth addressed immediately. she will need extractions. She will also need her heart murmur evaluated via an echocardiogram
