My name is NALA. My Animal ID # is A1090006.
I am a spayed female red and white akita mix. The shelter thinks I am about 8 YEARS old.

I came in the shelter as a OWNER SUR on 09/16/2016 from NY 10303, owner surrender reason stated was PERS PROB.

MOST RECENT MEDICAL INFORMATION AND WEIGHT
09/21/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 124.0 LBS.

S: geriatric exam; as per surrendering owner, Nala has hypothyroidism, arthritis, skin allergies, and is recovering from lyme disease O: BAR muzzled for exam, no oral exam OU- clear AU- clean no murmur, regular rhythm lungs clear and eupneic abdomen- tense, difficult to palpate BCS 7-8/9; ambulatory; no pain on palpation of limbs; possible stiffness on extension of right hip a few epidermal collarettes on medial thighs A: overweight hx of skin disease per previous owner- mild dermatitis at medial thighs seen today hx of lyme disease per previous owner hypothyroidism per previous owner arthritis per previous owner P: continue meds; consider placement for follow-up on possible lyme disease and hypothyroidism

09/16/16 14:34 Basic Information: Nala is an 8 year old female Akita that has several medical issues such as recovering from lime disease, skin allergies, and arthritis. Previous owner received Nala from a breeder as a puppy but can longer have her due to moving and is unable to take Nala as well as all of Nala’s medical issues Sandra cannot financially support her anymore. Socialization: According to the previous owner, around strangers, Nala does not like strangers especially men. Around previous owner & family, Nala is respectful and relaxed. Previous owner did not play with Nala. Nala does not do well children. Previous owner stated that Nala has never interacted with another dog or cats because she was worried about her reaction. She has never bitten a person or another animal. Behavior: Previous owners stated that Nala doesn’t have any behavioral issues but stated that Nala is very territorial. Nala isn’t bothered when any food, treat, or toy is taken from her except if she has a bone with meat then she will growl with hackles raised and may even bite. Nala is afraid of baths but enjoys having her coat brushed. Previous owner never trimmed Nala’s nails personally so it is unknown how she would react. Nala would bark when an unfamiliar person approaches the home, yard, previous owner, or a family member. For a New Family to Know: Nala is described as mellow, affectionate, and independent with a low activity level. When in the home, Nala is the type to follow you around seeking attention or will at least be in the same room. Nala favorite activity is to go for walks. Nala was kept mostly indoors and was fed dry Royal Canine dog food. According to previous owner, Nala is house-trained and would only have accidents if a relief walk is late. Nala usually goes to the bathroom on the grass. When left alone in the home or in the yard, Nala would be well-behaved. Nala knows commands sit, come, down, stay, paw, and “excuse me”. For exercise, she enjoyed slow walks with the previous owner & being in the yard. On the leash, Nala can pull hard and was never let off the leash.

Bite history: if has a bone with meat on it may bite Housetrained: is house trained and would only have accidents if a relief walk is late, usually goes to the bathroom on the grass Energy level/descriptors: low activity level Other notes: owner states does not like strangers, especially men SAFER ASSESSMENT: 9/20/2016 Look: 1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash slowly, body soft Squeeze 1: 5. Dog bites. Toy 1. No interest. Summary: Nala was relaxed and neutral throughout assessment until paw Squeeze, then bit assessor’s forearm without much warning. She did not break skin but left a bruise, bite and release. a little nervous, muzzle used for exam microchip scan negative spayed female approximately 8 yrs old, geriatric ears are clean nuclear sclerosis OU moderate dental tartar, dark pigmented gingiva 124lbs; BCS: 4.5/5, overweight pressure sores on forelimbs well groomed hair coat As per surrendering owner, Nala has hypothyroidism, arthritis, skin allergies, and is recovering from lyme disease. She came in with medication prescribed by previous veterinarian: L-Thyroxine 0.6mg 1 tab. BID Tramadol 50mg 3 tabs.TID Clindamycin 300mg 2 caps. BID Doxycycline 100mg 2 tabs. BID Vet Exam is scheduled on 9/21
