Medical Summary 10/15/16 MC scan NEG S/O: came with phenobarbital 32.4mg/tab give 1 tab bid x 28 days was set up–meds in safe BAR, whining amb x4, BCS 3/5 EENT: heterochromia, mild tartar and gingivitis ABN: SNP NEURO: NSF H/L: NSF LN: NSF A: h/o seizures: r/o intracranial vs. extracranial causes P: con’t with phenobarbital, seizure watch rec cbc/chem/phenobarbital level at 14 days of medication (unsure when started), neuro consultation and possible MRI, otherwise if responds to phenobarbital can con’t but will never know underlying cause. prognosis: fair to good with appropriate treatment ***emergency valium dose: 1.6ml IM, IV, or 3ML RECTALLY
