Medical Summary scan negative female intact S/O-QAR, pink mm, hydrated EENT-mild d/c AU, crusted d/c OU, crusted shut OD, blepharospasm OU once crusts removed, no nasal d/c PLN-all wnl H/L-no obvious m/a, clear and eup GI-mild dental tartar with some staining, otherwise negative oral, TNP GU-F, mgts of right 4th mammary gland (0.5 x 0.5 inch), no vaginal d/c MSI-BCS 3/5, amb x 4, pigmentation and scab like lesion approximately 4 inch x 1 inch with alopecia, erythema and alopecia on left flank approx 4 inch x 2 inch, otherwise ok haircoat Neuro-QAR, difficult to assess ocular related nerves due to crusting, normal facial sensation and motor, amb x 4 with hindlimb ataxia, no obvious pain on back palpation A-5 yo F Shih Tzu 1. conjunctivitis 2. skin changes R/O topical burn vs dermatitis vs other 3. hindlimb ataxia 4. mgt P-Rabies RHL SQ CIV LHL SQ DA2PP RFL SQ Bordetella IN Activyl <14 # applied monitor appetite and gait recommend bloodwork and CXR once placed and if no obvious mets, recommend sx removal of mgt TAB OU BID x 10 days vet check in 5 and 10 days
