DVM Intake Exam; Estimated age: 4 years; History : stray; Subjective: Observed Behavior – friendly; Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mild dental calc; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: Female, no spay scar seen; MSI: Ambulatory x 4, skin free of parasites, no masses noted, rough haircoat, a few papules at medial thighs, epidermal collarettes and some hair loss at caudodorsum; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; dermatitis- R/O underlying atopy vs food allergy vs mites (skin scrape negative) vs other; Prognosis: good; Plan:start simplicef 150 mg PO SID x 21 days; start diphenhydramine 50 mg PO BID x 14 days; SURGERY: Okay for surgery

Look: 1. Dog’s eyes are averted, with tail wagging and ears back. Allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth.
Tag: 1. Dog follows at the end of the leash, body low and a bit fearful.
Paw squeeze 1: 1. Dog gently pulls back his/her paw.
Paw squeeze 2: 1. Dog gently pulls back his/her paw.
Toy: 1. Minimal interest in toy, dog may smell or lick, then turns away.
Summary:Moon entered the assessment room with a soft, loose body. She was friendly and social with both handlers but her attention regularly was directed towards the door that led outside. Moon also laid down in between each assessment item but was easy to re-position.
Summary (1):3/20: When off leash with a large male, Moon was polite when greeting, comfortable with face to face interactions but did not engage in play when it was offered. Moons will also explore the yard with the company of the male dog.
