Medical Summary 01/31/17 Microchip scan pos #0A01426659 S:MI, approx. 4 years. Veterinary records from 1/31/17 found: KCS OU – STT = 0mm/min. Exposure keratitis with slight uptake of fluorescein stain OU. Otitis externa AU. Chronic skin disease, with negative skin scrapes. O: BARH. mm=pink, moist, CRT<2 s. Calm, friendly, easy to handle. ORAL: Minimal calculus. EENT: OU – mucoid discharge, scleral injection, diffuse corneal opacification. Pos. menace OU. AU- Mod. canal stenosis, thickening, yellow/tan discharge, and erythema. H/L: Lungs clear, no murmurs/arrhythmias. ABD/UG: Palpation WNL. Intact male, both testicles palpable. MS: BCS=5/9. Amb x 4. INTEG: Dorsum of head – thin fur, scabbing, erythema. Mod. lichenification, alopecia, erythema, moisture, and odor over: ventral neck, axillae, abdomen, scrotum, perineum, caudal forelimbs, medial hind limbs, and ventral thorax. Diffuse dry seborrhea over dorsolateral body. A: Age est. (based on good dental condition) 4 years. KCS OU. Chronic otitis externa AU. Chronic dermatitis with superficial pyoderma and malasezzia infection. R/o underlying allergy – atopy, food, fleas. P: Convenia 80 mg/ml. 0.6 ml SQ. Ketoconazole 200 mg 0.25 PO q 24 hours x 1 week. If doing well, continue for a full month. Optimmune OU BID long term. TAB ophth ointment OU BID long term. Otibiotic 3 drops AU BID x 10 days. Clean ears q 24 hours x 10 days. Will need long term therapy for KCS. May also need long term therapy for chronic ear and skin conditions, and evaluation for underlying allergies.
