DVM Intake Exam; Estimated age: 4; Microchip noted on Intake? n; History : stray. Reported chasing after children and parents threw object at left front leg causing superficial injuries. Subjective: BARH; Observed Behavior – allowed for PE although tense; Objective T = -; P = wnl; R = wnl; BCS = 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Teeth in good cond ; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Two descended testicles ; MSI: Two 1 cm long lacerations through the skin and sub-q above the left carpus. Patient is still weight bearing and allowed for cleaning with peroxide. Wound does not appear to be affect the muscle layer. After patient was walked wound continued to bleed. Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: externally normal; Assessment: Left front-limb laceration (x 2); Prognosis: Good; Plan: Give all intake treatments. Neuter. ; SURGERY: Okay for surgery ; Laceration – Sedate, explore and clean ; 0.6 ml butorphanol IM 0.6 ml Dexmeditiomidine IM – after the procedure 0.6 ml antisedan was given at 315pm
