DVM Intake Exam; Estimated age: 12yo; Microchip noted on Intake? neg; History : O/S; Subjective: BAR, sweet and friendly, well behaved during PE and med handling; Observed Behavior -; Objective ; T =; P = wnl; R = wnl; BCS 4/9; EENT: OU nuclear sclerosis and iris atrophy, ears clean, no nasal or ocular discharge noted; Oral Exam: minimal staining and calculus; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: FI, no MGTs, no vulvar dc; MSI: Ambulatory x 4, stiff strides and crepitus with HL’s, fleas, FAD on caudal thighs and base of tail. No masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally; Assessment; Geriatric; Intact female; Fleas; FAD; OA; Prognosis: good; Plan: ctm until transfer/adoption; SURGERY: Permanent waiver due to old age
