03/14/16 : S/O: QAR, standing with a hunched posture, mild dehydration, walking with long-strided hind limb ataxia, finches sin but does not vocalize on palpation of thoracolumbar spine – possiby painful, urinarted when picked up and on gental abdominal palpation – suspect urinary incontinence, nuclear sclerosis OU, severe dental disease, heart and lungs WNL on auscultation, abdomen soft and non-painful, ears clean AU, no CIRDC signs. A: hind limb ataxia: r/o trauma vs. IVDD vs. degenerative vs. neoplasia vs. other; geriatric; severe dental disease; urinary incontinence. P: monitor in medical – strict cage rest (can carry outside to eliminate), make sure eating and comfy. start rimadyl 12.5 mg PO BID x 7 days; start buprenorphine 0.22 ml BID x 7 days; start LRS 150 ml SQ SID x 3 days; check bladder TID and express as needed.
recommend new hope placement – rec neurologist consult, ataxia may improve with weeks of strict age rest and pain medication if caused by IVDD but ataxia may also worsen, best if MRI performed to determine underlying cause of neurologic signs
3/15/16 : Hx: Ataxia, urinary incontinence, possible IVDD. S: Quiet, allows all handling. O: QAR-H, MMs pink and moist; EENT: No discharge OU, AU, nose. Severe tartar, gingivitis and gingival recession. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Full, moderately firm, pt sometimes jumps away when abd is palpated. M/S/I: Initially would not walk or stand. Then suddenly jumped up and ran away. Walks intermittently, sometimes sinks to the ground. Hops or jumps up and moves rapidly in response to being touched on the caudal half of the body. Neuro: Ataxic – unbalanced, crossing over midline, not moving in a straight line. When carried to a table, moves front legs onto the table appropriately, but does not do this with hind legs. Weak but present patellar reflexes. Does not appear to respond consistently to painful stimuli in hind legs – does not respond to toe pinch but does pull feet away from a hand grasping the whole lower leg. UG: Female. Urinates in response to heavy abdominal pressure.
A:
- Ataxia – R/O traumatic spinal cord injury vs. IVDD vs. other
- Full bladder, difficult to express – R/O neurologic cause
Short-term prognosis: Poor
Current surgery status: Permanent waiver due to age.
P:
- Continue supportive care and cage rest
- Continue expressing bladder regularly
- Seek placement ASAP
