Medical Summary 05/29/17 11:34 DVM Intake Exam Estimated age: 14-16 yo Microchip noted on Intake? yes 276 098 100 642 037 History : stray foung in hotel with Sally A1113515 Subjective: Observed Behavior – nice calm allos full exam with no restraint Evidence of Cruelty seen – abandoned in hotel, skin disease and ocular disease Objective T = P = 120 R = 24 BCS 6/9 EENT: OU purulent discharge and pigmentation covering corna and medial sclera, AU purulent discharge and stenosis no nasal discharge noted Oral Exam: oligodontia & significant tartar and calculi PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, inspiratory stertor, cough on tracheal compression ABD: Non painful, no masses palpated U/G: intact female, flaccid slightly distended mammaries, no masses palpable, slightly enlarged vulva, no discharge MSI: Ambulatory x 4, no masses noted, moderate mm atrophy, l;ichinefication over ventral neck, thorax and abdomen, overgrown nails CNS: mentation appropriate, limited vision (no menace, unable to asess PLRs Rectal: full anal gg with thickned exudate Assessment KCS vs other pug keratitis vs other otitis externa vs otitis interna/media collapsing trachea vs other dermatitis r/o atopic vs inx vs other Plan clean ears Baytril Otic AU SID x 7 days TAB ophthalmic OU BID x 7d medicated baths 2x/week Prognosis: fair, recommend CBC./CHem/UA & Rads SURGERY: Permanent waiver due to geriatric age
