DVM Intake Exam; Estimated age: ~10yrs; Microchip noted on Intake? no; History : Owner surrender, brought in by field; Subjective: Observed Behavior – QARH. Allowed all handling; Objective ; P = WNL R = WNL BCS 5/9; EENT: nuclear sclerosis, ears clean, no nasal discharge noted; ral Exam: moderate dental tartar/gingivitis, 0.5cm mass at right upper lip buccal side ; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Female; MSI: Ambulatory x 4, grade 3/4 MLP on right side, skin free of parasites, open wound just lateral to right cranial mammary gland; abscess ruptured just below right medial canthus in three places, (3) 1cm areas of alopecia with no underlying dermatitis at right flank, right cranial dorsum and left flank; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric, R/O tooth root abscess, R/O mammary gland tumor vs other mass, luxated patella, alopecia; Plan: Rec CBC/Chem, chest radiographs, scheduled for tomorrow. Convenia 0.5ml SQ given. Continue to monitor while at BACC; Prognosis: poor; SURGERY: Permanent waiver due to age
