Medical Behavior Evaluation BLUE
Medical Summary Scan negative. Intact male. History : The patient was a stray dog. No vomiting, diarrhea, coughing or sneezing. Eyes seems to be dry. Calm if eyes are not manipulated. BCS 6/9. EENT: Cornal leceration, corneal neocascularization, conjuctivitis. No nasal discharge noted. Oral Exam: dental tartar. PLN: No enlargements noted. Normal size and shape. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated. U/G: Intact male. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities. Rectal: Not performed. Diagnostics- fluorescein stain- -ulcer in both eyes. Assessment : Corneal ulcer, KCS. Plan : HW negative. Pyrantel give po. activyl applied on skin. Eyes were flushed and cleaned. Optimune was applied OU. ( BID OU). Neo poly bac eye ontment applies OU- ( BID OU for 10 days).
PROFIE :
Mojo is a 7 year old male, white and black Chihuahua mix, he came to his family from a previous owner, and was surrendered because of them owner eviction. Mojo has a possible eye infection but has no known or injures, and has not recently been seen by the vet.
Socialization:
When Mojo is approached by a stranger he is shy a first and growls. He has lived with three children and is very relaxed and playful around the children. Around other dogs Mojo I relaxed and playful. When Mojo comes across a cat he growls and chases after them. Mojo has no bite history.
Behavior:
During Storms or fireworks, Mojo will hide under the bed. If an individual tries to touch Mojo’s food bowl he isn’t bothered and allows it to be removed. Mojo loves bath time and enjoys being in the water with his toys. He isn’t bothered when he is being held, and enjoys being brushed. When an unfamiliar person or family member comes to the house or approaches his owner Mojo will bark and bares teeth.
For a new family:
Mojo was described as playful, affectionate, and shy by his previous family.
He has a high activity level and often enjoys, fetch with tennis balls. His favorite activity is to lie down and watch tv. Mojo is feed house food 3 times a day and mostly stays indoors. Mojo is house trained as uses the potty on cement and grass. Mojo has never been crate trained and is always off- leash and stays close to his owners.
Admission behavior:
When the Counselor approached Mojo he grew tense, growled and bared teeth. The counselor asked the owner to collar and scan Mojo. Bedding was used to pick up Mojo for a photo
PLAYGROUP:
Mojo’s previous owner reported that he is relaxed and playful around other dogs.
3/5: When off leash at the Care Center, Mojo greets other small male and female dogs but mostly keeps to himself, exploring the environment.
