Hx: Geriatric exam S: Quiet, scared, alows all handling O: QAR-H, BCS 5/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. SEVERE tartar and gingival recession, incisors and canines are exposed up to the root and incisors are loose. PLNs: Mandibular LNs subjectively enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Sightly tense, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male intact, testicles soft and symmetrical. A: Severe dental disease, most teeth need to be extracted Short-term prognosis: Good with dental cleaning P: Reccomend immediate dental cleaning and radiographs, with extractions as indicated by rads 1088

07/30/16 10:42 Basic Info: Mojo is an 9 yr old male Chihuahua mix who is being surrendered due to his owner not being allowed to have him in her new apartment because he barks too much. He was not seen by a veterinarian and has no known health issues. Socialization: Around strangers, Mojo is friendly/outgoing. He has been around children and plays exuberantly with them. He is friendly with other dogs and cats and he has not bitten anyone or any other animal. Behavior: Mojo used wee wee pads in the home and was walked every so often. During storms/fireworks he’ll bark. He isn’t bothered by the following: having his food/food bowl, toys/treats touched/moved, being disturbed while sleeping/resting. Mojo loves bath time and being brushed. He hasn’t had his nails trimmed. If someone approaches the home/family member he wants to play. For A New Family to Know: Mojo is friendly, affectionate, playful, confident, excitable & independent with a medium activity level. The owner likes how friendly he is. He is mostly an indoor only dog who sleeps on in his dog. He eats dry dog food 2x a day and likes any kind of dog treats. Intake Behavior: Mojo allowed all handling. He allowed to be scanned (-), collared & taken pictures of. He was then transferred into a kennel with ease.

Mojo came into our center as an owner surrender citing housing issues. It is reported he has been around children, plays exuberantly, reportedly friendly with other dogs and cats. Mojo is reportedly housebroken to use wee wee pads indoors, walked every so often. He is reportedly not bothered if anyone touches his Food, Toys, Treats. Mojo is described as friendly, affectionate, playful, confident, excitable, and independent. During intake, Mojo allowed all handling. He held gaze for three full seconds during Look item. Mojo leaned into assessor’s hand during Sensitivity item, mouth closed, body soft. He followed at the end of the leash for Tag game, approached assessor at end of game. Mojo did not pull his paw back on both Squeeze attempts. He showed no interest in Toy. When off leash with other dogs Mojo offers brief sniffs, then becomes more preoccupied with wandering around the room. The Behavior Team feels Mojo would do well with an Average adopter. He may initially do well with a slow approach/slow introduction as well as first going home to a calm, quiet, structured environment so he can relax and know what to expect. Look: 1. Dog holds gaze with soft eyes, soft body. He allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog leans into assessor’s hand and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash, body soft Squeeze 1: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are relaxed Toy 1: No interest. Dog-dog: When off leash with other dogs Mojo offers brief sniffs, then becomes more preoccupied with wandering around the room.
