DVM Intake Exam:Estimated age: 8. Microchip noted on Intake? y History : owner surrender. Subjective:bar Observed Behavior – amenable to all aspects of exam, sweet and friendly. Objective:   T = P = 110 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted. Oral Exam: severe tarter and dental dz, 5-8 yrs based on dentitia PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated  U/G: fi, palpable mammary mass left 2nd to caudalmost mg 1/2 cm and soft mammary mass caudalmost mammary gland, fatty feeling MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, grade 3/4 mpl bilaterally CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal:   Assessment: 5-8 yr fi pom. mammary tumor left side last 2 glands – r/o benign vs malignant. dental dz  grade 3/4 mpl  Prognosis: good w/ removal of tumors Plan: rec ovh and removal of mammary masses last 2 on left side
rec: full dental, jt support supplementation. SURGERY:  Okay for surgery
