Allows all handling, attention seeking, calm Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Minimal dental tartar (may have had cleaning) PLN: No enlargements noted H/L: Grade 2-3 heart murmur, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Heart murmur Plan: Continue to monitor while at BACC. Rec echocardiogram with placement Prognosis: Good SURGERY: Permanent waiver due to age and heart murmur

PROFILE:
Mocha is an 8 year old small tan male dog who was surrendered by his owner due to the fact she can no longer have pets in the home. The owner stated that she got Mocha as a gift and she has had him since he was 8 weeks old. The owner stated that Mocha has no known medical issues.
Owner states that when around strangers Mocha is shy for a few minutes but will warm up. He has spent time around children and he is relaxed and playful. When playing with children and adults he is gentle. He has not spent time in the home around other dogs or cats so it is unknown how he will react.
Owner stated that overall Mocha behaves well. During fireworks or loud noises he isn’t bothered. Mochas is friendly if someone was to touch his food treats or toys. He isn’t bothered if he is restrained pushed off the couch or disturbed while he sleeps. Mocha will tolerate being bated and brushed. He has only had his nails trimmed by the groomer so it is unknown how he will react. Mocha will bark if someone unfamiliar approaches his house or family member.
Owner described Mocha as friendly affectionate and playful with a medium activity level. Owner stated that since Mocha has gotten a bit older he doesn’t have much interested in playing with toys. He loves to cuddle and be pet. Mocha has been kept mostly indoors and eats dry food. He is very much house trained and rarely has accidents. When using the bathroom he will go outside on the grass, cement or on a wee wee pad in the home. Mocha has never been crated or left in the yard so it is unknown how he will react. Mocha knows how to sit, give paw and come when called. He is used to brisk walks on the leash. When off the leash he will wander but will come back when called.
PLAYGROUP: Mocha’s previous owner reported that he has not spent any time around other dogs.
9/10: When off leash at the Care Center, Mocha greets other small male and female dogs but mostly keeps to himself while exploring the yard.
