Medical Summary 02/25/16 14:24 Scan : negative Sex: spayed Approx 6 yr History of recent pancreatitis diagnosed at veterinary hospital. Dog was provided supportive care and owner declined hospitalization and reports minimal improvement, not eating or drinking currently. S/O: BAR, no ocular or nasal discharge. Clean ears. Dental disease grade 4/4, significant calculus buildup. MM are pink and moist, CRT <2s. Euhydrated. BCS 7/9, overweight. NOrmal cardiac auscultation. Abdomen tense and painful on palpation, whined. Slight distension. Ate I/d canned food readily. Allowed all handling NOSF A: Acute pancreatitis, dental disease P: Will start Buprenorphine for pain and feed I/d diet only. Will monitor for vomiting and stool quality. SQ LRS, cerenia, famotidine, or metronidazole maybe indicated based on clinical signs. Recheck daily. Will request NH rescue plea for placement and follow-up care. Moca may require hospitalization and supportive care if his condition declines from current presenation. A follow-up C-PLI snap may indicated to record improvement. PROGNOSIS: GOOD
