Medical Summary Microchip: negative Sex: intact female Age: appx 14-16y Mentation: QARH Eyes: n.scler, difficulty focusing on objects Ears: heavy waxy d/c, cleaned most of inner pinnae but outer also appears to have caked wax (gentle novalsan scrub) Nose: no d/c Teeth: severe dental disease (heavy calculus, swollen gums, missing teeth, etc) If abnormal BCS: underweight and muscle wasted Skin: WNL Hair Coat: WNL Declawed: N/A, overgrown, trimmed nails Any injuries: no; possible tooth root infection on left side of upper arcade (swollen area under left orbit, soft, tender) and slight swelling along muzzle; arthritic gait, unable to sit comfortably, leans against objects when walking Behavior: painful but allows handling, unaware of surroundings at time Medication: N/A, preventatives given
