I came in the shelter as a OWNER SUR on 07/23/2017 from NY 11233, owner surrender reason stated was PERS PROB. I came in with Group/Litter #K17-105229.

DVM Intake Exam Estimated age: 4 Microchip noted on Intake? neg History : o surrender both dogs (A1119460, A 1119458) – moving Subjective: QAR-H, shaking head, painful ears Observed Behavior – Easily handleable, allows all exam and ear cleaning Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = 120 R = eup BCS= 5/9 EENT: Eyes clear. Ears – marked brown d/c AU, mod erythema AU, L pinna thickened and irregular with marked hyperkeratosis, NND. Oral Exam: MMpk. Mild tartar, miniml gingivitis. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male – 2 scrotal testicles – WNL. Pre-scrotal area cln. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: n Assessment: Otitis externa – acute infection with marked chronic changes Ears pruritic. 07/23/17 14:24 Basic Information: Mister is a 6 year old unneutered male that has no past or current medical issues that the owner is aware of. Owner has had Mister for 3 years but had to surrender due to personal problems. Socialization: According to the owner, around strangers Mister is friendly and loves to jump and say hello to everyone. He has been around owner’s nieces and nephews ages 6-14 and was always relaxed and respectful around them. Owner had 2 other dogs in home, one who is Mister’s mother A1119458 and a sibling and owner stated that Mister would always eat with his mother, play and snuggle with her . Around his sibling, he was always relaxed and respectful. Owner did not have any other pets at home and stated that when Mister was taken on a walk and saw cats, he was relaxed and tolerant. Mister does not have a bite history. Behavior: Mister is not bothered by fireworks or thunderstorms, having his food, treats or toys taken away, being pushed off furniture, being held or restrained, being given a bath, having his nails trimmed, having his coat brushed, or being disturbed while sleeping. When unfamiliar people would come to the home, Mister would bark at first. For a New Family to Know: Mister is described as friendly, affectionate, playful and excitable. He had a medium activity level and enjoys playing balls, stuffed animals and ropes. In the home, he would constantly follow the owner around from room to room. Owner fed Mister both wet and dry food brand Beneful and Purina along with rice, chicken and steak. Mister is house trained and would only have an accident if he was sick. When owner would leave him alone in the home, he was always well behaved. Owner had a crate for Mister but kept it open and stated that Mister would sleep in it from time to time. He knows the cues sit and come. He also responds to have a seat, knock it off and you know better. For exercise, Mister was taken on brisk walks on the leash or on off leash walks in the park. When on leash, Mister tends to pull very hard. Owner has never tried to walk him off leash as he would run away. Behavior during intake: Mister had a loose and wiggly body during intake and was jumping on the counselor during intake to give kisses. Counselor was able to scan for a mc, collar and take a picture without any issues.

Mister’s previous guardian reported that he was relaxed and playful around the other dogs in the home. 7/24: When off leash at the Care Center, Mister greets the novel female with a slightly nervous posture, hackles raised, body slightly tucked. He softens up as the session continues, mostly exploring the yard and occasionally checking in with the other dog. 7/25: Mister displays a loose body posture when greeting a novel male. While keeping to himself, he investigates the pen and its smells.
