I came in the shelter as a STRAY on 06/12/2017 from NY 10029, owner surrender reason stated was STRAY.

06/13/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 14.0 LBS.
Hx: dx with heart murmur and oronasal fistula on intake yesterday; had brief episode where he fell over-was suspected to be a syncopal episode but today he appears ataxic so considered more likely to be due neurologic disease at this time CBC-mild basophilia, significance unknown Chem-mild elevation of ALT (279), mild hypocholesterolemia (98) S/O BAR, friendly and energetic mm pk/pigmented, oronasal fistula noted on R side nose clear grade IV/VI heart murmur eupnic, normal lung sounds in all fields soft abdomen, no palpable masses mild proprioceptive ataxia, does occasionally lose balance and falls decreased-absent proprioception on RF, mildly delayed on RH; normal on L side mild dec withdrawal x 4 resistant to turn neck side to side, sensitive on palpation of cervical spine A elevated ALT-r/o acute vs chronic hepatopathy vs benign elevation vs paraneoplastic vs other proprioceptive ataxia, worse on R side-r/o cervical IVDD vs intracranial dz vs other heart murmur-r/o valvular vs other cervical pain-r/o IVDD vs referred pain P rimadyl 12.5 mg PO SID x 7 days rec’d cardiac work up ideally with CXR, blood pressure monitoring, ECG and echo-needs CXR at minimum to evaluate heart size rec’d neuro consult ideally with MRI rec’d AUS to evaluate liver rec’d dental if ok for anesthesia after diagnostics

06/12/17 13:02 Mister was really thirsty when he arrived to MACC thus we gave him a bowl of water. He sat nicely and had a relaxed body. He allowed the admissions counselor to pick him up to take his picture. His activity level during intake was low perhaps due to his age.

A volunteer writes: He looks like an expensive stuffed toy, so dandy. Yes, his given name suits him so well…Mister. Mister is nice and quiet in his upper kennel, all ready to be picked up for a walk around the yard. He does his business at once, goes down carefully the short flight of stairs and is a bit wobbly on the soft grass. He explores carefully the area, stops for some treats and caresses and does not mind to be lifted onto my lap. I love that Terrier face with dark big button eyes. And that wiry hair…Mister is really the cutest and so dear. Like many elderly, Mister comes with some medical issues that will have to be attended by his new owner or foster. This gentle and friendly little guy is so worth the care and your attention. Come and meet him soon at the Manhattan Care Center. A home sweet home and a person or family to love is all that Mister dreams of.

Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is closed. Tag: 1. Follows at end of leash, body soft. Squeeze 1: 2. Dog quickly pulls back. Squeeze 2: 2. Dog quickly pulls back. Toy: 1. Minimal interest, dog sniffs toy. Summary: Mister displayed no concerning behaviors on his assessment. DOG-DOG INTERACTION ASSESSMENT: Summary PLAYGROUP: 6/13: When off leash to other dogs at the shelter, Mister greets other dogs politely and wanders the pen. INTAKE BEHAVIOR Upon intake Mister had a relaxed body and allowed handling. MEDICAL BEHAVIOR: 6/12/17 During his initial medical exam, Mister was tense. ENERGY LEVEL: We have no history on Mister so we cannot be certain of his behavior in a home environment. In the care center, he displays a low level of activity.
