DVM Intake Exam:Estimated age: reported 20 yrs old, exam consistent with reported age Microchip noted on Intake? Scan neg- MC placed. History : OS, reported to have had multiple UTIs and been treated with antibiotics, now is urinating frequently in the house Subjective: QAR. Observed Behavior – Wagging tail, allowed all handling Objective: QAR, MM pink and moist, BCS 6/9 EENT: No nasal or ocular discharge noted. Nuclear sclerosis present OU. Mild waxy debris AU. Oral Exam: Moderate calculus observed on premolars and molars PLN: No enlargements noted H/L: 3/6 left apical systolic murmur auscultated. Honking cough when trachea palpated, upper respiratory wheeze on auscultation ABD: Non painful, no masses palpated U/G: Female, reported to be spayed MSI: Ambulatory x 4, skin free of parasites, ~5 x 5 cm SQ, soft mass associated with caudal right mammary gland. Matting of fur near eyes and around vulva. Urine staining and smell around hind end CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally
Assessment: 1. Geriatric- suspect underlying condition 2. Dental disease 3. Murmur- suspect mitral regurgitation 4. Urinary condition- UTI vs. urinary incontinence 5. Tracheal collapse 6. Mammary mass 7. Matting Prognosis: Fair Plan: Awaiting rescue SURGERY:
