DVM Intake Exam : Estimated age: 1-2 year Microchip noted on Intake? positive History : os Subjective: BARH Observed Behavior – very sweet but nervous-was muzzled for exam. Easily handleable. Did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective : P = wnl R = eupneic BCS 4/9
EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: limited oral exam due to muzzle PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated U/G: FI, no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, mild erythema interdigitally and on ear pinna, not pruritic CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Apparently healthy Mild dermatitis Plan: Continue to monitor while at BACC Start benadryl 2.2mg/kg PO BID x7d until 12/5 If no improvement with medication, rec dermatology consult but likely just contact allergy Prognosis: Excellent SURGERY: ok to schedule for surgery
